Tournament Sign-up Information

The Title Field -Enter your position with the team(Manager, Coach, Etc)

The Tournament Field- Select the tournament you wish to play in.

The Age/Sex Field -Select the Age & Sex of the age grouping you desire.

The Team Name- Enter the name of the Team (Note: if your group has teams with the Same Name include the Age/Sex at the End of the Name)

The Contact Name- Enter the Name of the person who will be responsible for this team, and will receive team communications.

The Street -Enter the street address of the contact.

The City- Enter the City address of the contact.

The State- Enter the State abbreviation of the State ex. (Pa.)

Zip Code- Enter either the 5 or 10 digit zip postal code.

Home Phone- Enter the home phone number format (xxx-xxx-xxxx)

Work Phone-Enter the work phone number format (xxx-xxx-xxxx)

Cell Phone- Enter the cell phone number format (xxx-xxx-xxxx)

E-Mail- Enter the complete email address of the contact person.

Entry Daye- by default todays date is entered.

Hit Submit- Click Submit to send in your entry. You will receive a message if you are successful. Follow the directions..
