Step #1 To Sign-up for a team or training time you must complete the following forms. The 1st step is to complete information that we need for scheduling .This form is required for everyone entering teams in a league or just buying training time.

Step #2 If you are entering a team in a Futsal League, ( Breakthrough League, or Fledgling League)? To complete the team sign-up you must first register your coach with USFF. This is because a current USFF Coach ID# is required to complete the process. This requires you to register your coach and pay the $12.00 USFF Registration Fee. After you have done so you can move on to step #3. (One Coach ID# can be used for multipule teams.) Please Goto Step #3

Step #3 If you are entering a team in a Futsal League, (, Breakthrough League, or Fledgling League) you now go on to 3rd tep to complete the actual USFF Team Registration. This is the final information you have to complete and submit to sign-up for a league. Tthese steps are required for teams entering a league no exceptions.

Help For Scheduling Form

The Title Field -Enter your Teams Club or group name,

The Team Name- Enter the name of the Team (Note: if your group has teams with the Same Name include the Age/Sex at the End of the Name)

The League Field- Select the League you wish to play in.

The Age/Sex Field -Select the Age & Sex of the age grouping you desire.

The Contact First Name- Enter the First Name of the person who will be responsible for this team, and will receive team communications.

The Contact Lastt Name- Enter the Last Name of the person who will be responsible for this team, and will receive team communications.

The Address -Enter the street address of the contact.

The City- Enter the City address of the contact.

The State- Enter the State abbreviation of the State ex. (Pa.)

Zip Code- Enter either the 5 or 10 digit zip postal code.

Home Phone- Enter the home phone number format (xxx-xxx-xxxx)

O-Phone-Enter the work phone number format (xxx-xxx-xxxx)

Cell Phone- Enter the cell phone number format (xxx-xxx-xxxx)

E-Mail- Enter the complete email address of the contact person.

Play Day #1- enter the 1st choice of day you want to play on

Play Day #2- enter the 2nd choice of day you want to play on( Required)

Play Day #3- enter the 3rd choice of day you want to play on( Required)

Special Requests-Enter any dates you must be off.

Timing request must be in the following format:

Between 7:00 am-1:00 pm or
Between 1:00 PM- 10:00 pm

Entry Date- by default todays date is entered.

Security ID- Enter the number "13579" (No Quotes)

Hit Submit- Click Submit to send in your entry. You will receive a message if you are successful. Follow the directions..

Look For the Confirmation In the Box Were the Form Was to Quarentee We got your entry.
